Mullerian Adenosarcoma of the Uterine Corpus



Abstract
Mullerian adenosarcoma was first reported by Clement & Scully(1974)", showing benign appearing neoplastic glands lying in a sarcomatous stroma. The glands were lined by a variety of mullerian epithelial cell types, and the stromal element resembled endometrial stromal sarcoma. Authors experienced a case of Mullerian adenosarcoma in the uterine corpus of a 57 years old female. She was complained of irregular vaginal bleeding for several months. Past-medical history showed no irradiation or operation history and specific medical illness. Physical examination showed uterine enlargement and mild tenderness on the pubic area. Laboratory findings including CBC, urinalysis and blood chemistry were within normal limits. Chest PA and electrocardiogram were nonspecific. Therefore, operation was done, at the second hospital day, under the clinical impression of leiomyoma. Operative findings revealed smooth surface of uterus and no adhesion of the surrounding tissue. Both adnexae were unremarkable. Total hysterectomy was done. The submitted specimen(O-85-522)consisted of a totally removed uterus, measuring 5.5 x 4.5 x 8cm. Uterine cervix showed focal erosion. Uttering cavity is largely obliterated by a pedunculated polypoid mass originated from the fundus, measuring 4 x 3cm. Myometrium was unremarkable. On section of the polypoid mass showed grayish white appearance and focal areas of myxoid degeneration. But neither hemorrhage or necrosis was seen. Histopathologically the polypoid mass showed two different neoplastic components composed of bening glanudlar structures and sarcomatous stroma. The epithelial components of glands are largely composed of resembling endometrial glands with atrophic,
proliferative and cystic change. Focally scattered are areas of stratified squamous epithelium and eosinophilic glandular lining cells. The intervening stromal tissue was partly myxoid, and most of them showed compact cellularity composed of pleomorphic spindle to ovoid cells with mitotic figures. With above features this case was interpreted as Mullerian adenosarcoma of uterine corpus.
